Individualized Home Supports (IHS) Services

We are constantly hiring caregivers for our Individualized Home Supports (IHS) Services. The IHS Worker is responsible for developing, implementing, and monitoring support plans based on company guidelines. Support plans are developed with the patient’s goals in mind such as:

  1. Skills teaching which includes budgeting, community integration, social skills, and housekeeping skills.
  2. Housing (subsidized housing search, completing applications, Section 8)
  3. Household Management (paying bills, completing and organizing paperwork)
  4. Nutritional Management (grocery shopping, meal planning, and preparation)

  • Qualifications: Applicants must meet one of the following criteria:

    • High School Diploma/CNA/HHA
    • US citizenship.
    • Education and experience relevant to the job responsibilities assigned to the staff and the primary disability of the persons served by the program.
    • Be able to demonstrate competence in the areas required by the Support Plan.

  • Other requirements: Applicants must be 18 years of age and have reliable transportation to meet and assist patients in accessing the community. Valid driver’s license with proof of insurance. Excellent writing and verbal skills.

    Excellent interpersonal skills and ability to work with a diverse population of people, strong organizational and problem-solving skills, and is able to pass DHS criminal background checks.

PCA/ Homemakers/ Respite Care/ Companionship Caregivers

We are hiring Caregivers for a variety of clients in the cities of our service areas.

  • Duties: Our Caregivers’ duties include bathing, feeding, toileting, grooming, dressing, light housekeeping, laundry, and other ADLs. Homemakers assist clients with housecleaning, laundry, meal planning/preparation, and sometimes, grocery shopping. You may be combining two or three services for some clients.

    Flexible 30 – 40 hours weekly but may vary depending on your availability and work ethic.

  • Qualifications: HHA/CNA/NAR or High School Diploma/GED. You must take and pass the mandatory DHS online Training. If you already have your certificate, then bring us a copy.

  • Experience: Prior nursing home, assisted living facility, group home or home care experience is a plus but not compulsory.

  • Requirements: Candidates must be able to pass criminal background checks as required by Minnesota health statutes. They must be dependable, possess good communication skills; be open to flexible schedules, and have reliable transportation with proof of insurance.
